Established patients at Form Chiropractic are welcome to walk in during business hours, Tuesday to Saturday, at our downtown Kansas City clinic. New patients need an appointment: book online through Jane or call (816) 673-1188. Once you are scheduled, Jane emails your intake forms so you can fill them out before you arrive. Your first visit is a consult, exam and treatment, and takes about 45 minutes.

Walk-in visits are for established patients, meaning anyone Dr. Smith has already seen at Form Chiropractic. Come in during business hours at 600 Broadway Blvd Suite 175 (downtown Kansas City) and check in at the front desk; walk-ins are worked in around scheduled appointments. If you have not been to Form before, book your first visit online through Jane or call (816) 673-1188. Jane emails your intake forms once you are scheduled, and the first visit covers a consult, exam and treatment in about 45 minutes. We accept most major insurance plans (BCBS, Aetna, Cigna, Humana, UHC, Medicare, UMR, VA, GEHA, Gravie, Surest) and offer affordable self-pay rates if you are uninsured.

Established patients can walk in during business hours: Tuesday 9:30am to 1pm and 3pm to 6pm; Wednesday 9:30am to 1pm; Thursday 9:30am to 1pm and 3pm to 6pm; Friday 9am to 12pm; Saturday 9am to 1pm. Closed Sunday and Monday.
Will I be seen by Dr. Tyler if I walk in?
Dr. Tyler Smith sees the majority of patients personally. If he is fully booked, the front desk will let you know upfront and offer either a wait or a return time that works.
